Patient Forms
Please complete the forms below online so we can expedite your first visit. You will be asked to upload the front and back of your insurance card and prescription card (if applicable), along with a valid photo ID such as a driver’s license. A credit card is required to keep on file when scheduling your initial appointment.
To begin, click the New Patient Packet link below. Once your submission is received, our scheduling team will contact you to arrange your appointment.
Our office accepts checks; however, a $50 fee will be applied to any check returned for insufficient funds.
Our practice is in network with CareFirst/Anthem, Aetna, Medicare, and Tricare. We are happy to see patients with other insurance plans, but we do not file claims to out‑of‑network carriers. Out-of-network claims should be submitted by you, and our office will supply a detailed receipt of the visit upon check-out. We do not accept any Medicaid plans, including those used as secondary coverage or in dual‑enrollment arrangements—even when a commercial plan is listed as the primary insurance. Due to regulatory requirements, our practice is not permitted to see patients who have Medicaid in any capacity.
What To Expect
Being well-prepared for your appointment will ensure that you are seen on time and that the doctor has all of the needed information to provide the best possible care for you. It will also help alleviate any unnecessary anxiety you may be feeling prior to your first appointment. Please take some time to review our staff page and familiarize yourself with our doctors. We look forward to your first visit. Please arrive at least 10-15 minutes earlier than your initial appointment time.
